When you click on the button it will come up with the following screen

which is the Depression Test Screen, as you can see there are 25 questions to answer to give you your depressive score. Once you have answered the questions it will allow you to stamp your score into the diary to see how you progress over the weeks. All you have to do is click on the "ADD Score TO DIARY" button.
